Dr. Jason Gospodarek, OD

Dr. Gospodarek earned his Doctorate in Optometry with Honors from Indiana University–Bloomington, after a Bachelor of Science in Biology with a Chemistry minor from UW–Stevens Point. His clinical training took him through four practices across the Midwest and South — and then he came home to Northeast Wisconsin, where he's cared for Green Bay's eyes ever since.

In 2019 he was voted Doctor of the Year across the entire nation among the optometrists in his national optical network — recognition from peers for the kind of careful, personal exams his patients already knew.

The practice operates independently inside the Green Bay Sam's Club — you don't need a membership to be a patient. What you get is a private-practice standard of care with everyday convenience.

Same doctor, every time

No rotation of strangers. The doctor comparing this year's scan to last year's is the one who took it.

60 days of follow-ups

Complimentary follow-up visits for 60 days per diagnosis — because care shouldn't end when the exam does.

Insurance made simple

We may accept insurance plans for exams; when a plan isn't accepted, you get a super-bill to submit for reimbursement.

No membership required

Sam's Club membership is never required for eye care. Members may get discounts on glasses and contacts.
